2012 School Census
Port Jervis School
District asking residents to complete census

The Port Jervis School District is
counting on you! In accordance with New York
State Department of Education requirements, the school district
is taking a census of all residents with children - and the
district needs your help.
The purpose of the census is to assist the district in planning
for future student enrollment. In an effort to keep costs to a
minimum, the district is conducting the census online. This will
eliminate the additional costs of printing and mailing thousands
of census forms.
District residents with children between the ages of 5 - 18 are
asked to take a few minutes to complete the online census. All
information will be held in strict confidence.

A printed version is also available
To accommodate residents who do not have access to a computer,
paper copies of the census are available at the District
Administration Building, located at 9 Thompson Street. If you
want us to mail you the census form, please call Lisa Buczek at 858-3100,
Ext. 15533. You can also download a printed version by
clicking here.
If you choose to
complete the paper form, please return it by August 31 to:
