Port
Jervis must submit student weight/health data
this year to NYS Health Department
New York State Education Law requires
districts to weigh and measure students during their required
school health examinations. These numbers are used by the school
doctor and nurse to calculate student body mass index (BMI) and
determine if a student's weight is in the healthy range for
his/her height and age group.
Every year,
school districts are selected to submit an aggregated summary
report of their students' weight status
group to the NY State Department of Health. Port Jervis was
selected for the 2011-12 school year to provide a summary report for students who
were in grades kindergarten, 2, 4, 7 and 10 in 2010-11.
The information
will help health officials develop programs that make it easier
for children to be healthier. Even though no names or individual
student information will be included, parents may choose to have
their child's data excluded from the survey report.
If you DO NOT wish to have your student’s weight status group
information included as part of the Health Department’s survey
this year, please submit the required release form to the school
nurse in your student’s school no later than October 31, 2011.
The form can be
obtained from your school nurse.
