 Safer Routes to School Survey

The Port Jervis City School District and Port Jervis Police Department are working together
to seek funding from the federal Safe Routes to School program to provide students with safe, appealing routes to walk to school. The goal is to improve student safety and reduce traffic within a two mile radius of Port Jervis Middle School and Anna S. Kuhl Elementary School so that more students will walk or bike to school.

As part of the grant application process, the Police Department is seeking input from parents of students who live within walking distance to school to identify the types of road/sidewalk safety changes they feel are needed.

Parents have two ways to provide feedback for the Safe Routes to School grant:
Online Survey - Click here to access the online survey

Printed Survey - Click here to download the printed survey. Please return the completed survey to the District Administration Building, 9 Thompson Street, or your child’s school office. Completed questionnaires can also be faxed to 845-858-3252.

We ask that you please return/submit completed surveys by August 15.
